What Happens When You Stop Taking Weight Loss Medications (such as Mounjaro or Wegovy)?

It's one of the questions we get asked most often here at MSH Weight Loss, and it's a completely understandable one. Whether you're approaching your goal weight, thinking about a break, or just planning ahead - knowing what to expect when you come off Mounjaro or Wegovy can make a real difference to how well you protect your progress.
Here's what you need to know.
Your appetite will return
Wegovy works by mimicking the natural GLP - 1 hormone, Whilst Mounjaro works by mimicking two (GLP-1 and GIP). These hormones help regulate hunger and slow down digestion. While you're taking Wegovy or Mounjaro, food noise quietens down, you feel full sooner, and eating less feels manageable.
When you stop, that hormonal support fades. These medications have a half-life of around one week, so effects start to reduce fairly quickly. Most people notice their appetite returning within a few weeks - sometimes sooner. Cravings can come back, portions that felt satisfying may no longer feel like enough, and the mental effort of managing food intake increases again.
This isn't a personal failing. It's biology.
Weight regain is common - but not inevitable
The research is honest about this. A 2025 analysis of the SURMOUNT-4 trial, published in JAMA Internal Medicine, tracked 308 participants who stopped Tirzepatide (Mounjaro) after significant weight loss. Within one year, 82% had regained at least a quarter of the weight they'd lost.
That's a striking figure, and we'd rather you know it than not.
What the data also shows, though, is that people who had built solid habits during treatment - consistent eating patterns, regular movement, good sleep - tended to maintain more of their results. The medication works best as a foundation, not a ceiling.
Other health benefits can reverse too
It's not just weight. The same SURMOUNT-4 analysis found that improvements in blood pressure, cholesterol and blood sugar control also reduced in those who regained weight after stopping. The health gains from Mounjaro are real, but they're tied to keeping the weight off.
So what are your options?
You don't have to choose between staying on Weight Loss medications indefinitely or stopping completely and hoping for the best. There are a few routes worth considering:
Maintenance programme - A lower, less frequent dose to help you hold your progress while you build long-term habits.
